We are authorized to collect Personally Identifiable Information (PII) from you by You (Client) authorizing Us (Agent) to collect PII. Any PII we collect is used to help complete and submit your application, as well as to help determine eligibility for an applicable program. 

 

If you choose to give us PII, we may share this information with a carrier or underwriter, or an authorized agent for the determination of eligibility for a product or solution. PII is used or disclosed only under the following circumstances: determining eligibility for a solution or product.

 

Our request for the PII is voluntary. If you choose not to provide us with the PII requested, or not to respond to certain questions, your eligibility for a solution or product may not be able to be determined. The inability to determine eligibility for a solution or product may well render the application canceled or incomplete, voiding any service agreement or voiding the ability to complete the contracted service between Us and You.
